The Community Diabetes service provides short-term specialist expert advice to adults with a Solihull GP to manage their diabetes.

We provide the following services:

Group education sessions for Newly Diagnosed Type 2 Diabetes

A fun, interactive 2 hour session covering topics including:

  • What is Diabetes, what care to expect and avoiding short and long term complications.
  • The diet and lifestyle changes that help to manage Type 2 Diabetes

The X-PERT Course for Type 2 Diabetes

  • Learn more about how to manage your, food, medication and blood glucose levels in a relaxed, fun and hands on course.
  • There is plenty of opportunity to ask questions, as well as discussions to swap information and meet other people with diabetes.
  • There are separate courses for those on diet or tablet control and those taking Insulin, and are 2.5 hours a week over 6 weeks.
  • We also do a one off 2.5 hour ‘Carbs Matter’ session to learn more about carbohydrate foods and their effect on blood glucose.

The HET1E Course for Type 1 Diabetes

  • Learn how to count the carbohydrate in your food and adjust your insulin to match what you eat and your lifestyle.
  • Opportunity to spend time with others with Type 1 Diabetes and learn from each other’s experiences.
  • The course runs 1 day a week over 4 weeks.

Appointments with Diabetes Specialist Dietitians

The Diabetes Specialist Dietitian can provide advice on how diet and lifestyle can help to control your Diabetes.

Appointments with Diabetes Specialist Nurse

Your practice may refer you for an appointment with one of our Diabetes Specialist Nurses if you require specialist support with your diabetes control or treatment.

Appointments with the Intermediate Clinic

At the Intermediate Clinic our GPs with Specialist Interest in Diabetes (GPSI) or Diabetes Consultant can provide specialist support with your diabetes treatment.

Where are the clinics?

Clinics are available at differing times and locations around the Borough. These include Land Lane Clinic in Marston Green, Hurst Lane Clinic in Castle Bromwich, Grove Road Clinic in Central Solihull and Shirley Clinic.

How can I be referred into the Community Diabetes services?

A referral form is accepted from a health care professional (e.g. GP, Practice Nurse or Community Nurse), which clearly identifies the reason you are being referred.

If you feel you would benefit from the service and would like to know more, ask your GP or Practice Nurse/Specialist Nurse to refer you.

If you would like to attend the X-PERT program or Carbs Matter, you do not need a referral from your GP, but can contact the service and request further information that can be sent to you.
